Referrals
If your insurance company requires you to obtain a referral/authorization to see another physician, you must advise us at least seven days prior to your appointment. To obtain that authorization please call (305) 558-3571 for the Hialeah office, (954) 436-1927 for Pembroke Pines, and (305) 398-1991 for Doral.
Payment is accepted by cash, check, or Mastercard/Visa at the time of each visit. If you are unable to make payment according to this policy please speak with our office manager prior to this treatment.
There will be a $10 additional charge for any returned check.
